Smile Shares Memberships Basic Share
Annual membership is $299 for current patients, $399 for new patients (new patient fee includes comprehensive new patient exam and full mouth series of digital x-rays). The equivalent non-member value is $707 in regular fees. Basic Share includes:
- 2 cleanings
- 2 fluoride varnish treatments
- 1 oral hygiene coaching session
- 2 periodic exams
- Any needed x-rays
- Professional whitening for $200 (a 50-percent discount off the regular $400 fee) or 3 whitening touch-up boosters
- 10 percent off all treatment services available at Strong & Healthy Smiles (exclusive of lab fees)
- 1-year warranty on materials and workmanship
- Priority Emergency Care
Premium Share
Annual membership is $399 for current patients, $499 for new patients (new patient fee includes comprehensive new patient exam and full mouth series of digital x-rays). The equivalent non-member value is $1,115 in regular fees. Premium Share includes all of the services of Basic Share, plus:
- 1 additional cleaning
- 1 additional fluoride varnish
- 1 additional oral hygiene coaching session
- Free professional whitening ($400 value) or 3 whitening touch-up boosters
- 15 percent off all treatment services available at Strong & Healthy Smiles (exclusive of lab fees)
- 3-year warranty on materials and workmanship
Family Memberships
The first adult family member in a household pays for a full share; additional shares are available for family members (adult or child) in the same household as follows:
- Second family member gets a 25 percent discount on share cost (choice of any level)
- Third and more family members get a 50 percent discount of share cost (choice of any level)
- Share types (Basic, Premium, and Elite) can differ among family members within the same household
Elite Shares
Annual membership is $599 for current patients, $699 for new patients (new patient fee includes comprehensive new patient exam and full mouth series of digital x-rays). The equivalent non-member value is $1,323 in regular fees. Elite Share includes all of the services of Basic Share, plus:
- 2 additional cleanings
- 2 additional fluoride varnishes
- 2 additional oral hygiene coaching sessions
- Free professional whitening ($400 value) or 3 whitening touch-up boosters
- 20 percent off all treatment services available at Strong & Healthy Smiles (exclusive of lab fees)
